About This Service

What Are
Health and Disability Services?

Uni-Hope's Health and Disability (HD) program supports adults with physical disabilities and chronic health conditions in maintaining independence, safety, and quality of life at home. Our trained staff provide compassionate, medically-aware support that respects each person's dignity and follows their healthcare team's guidance.

We work closely with each individual's healthcare providers, service coordinator, and family to ensure every aspect of their care plan is followed consistently — so they can remain safely in the community rather than transitioning to a nursing facility.

Common Questions

Frequently Asked
Questions

Iowa's Health and Disability (HD) waiver provides Medicaid-funded community-based support services to adults with physical disabilities or chronic health conditions as an alternative to nursing facility placement.

Uni-Hope's HD support services are not medical services but are medically-informed. Our staff are trained to follow healthcare provider instructions, assist with medications, and recognize changes in health status.

Waiver eligibility is determined by Iowa DHS. Contact us or your case manager to discuss your specific situation and what combination of services may be available.
