Uni-Hope's Health and Disability (HD) program supports adults with physical disabilities and chronic health conditions in maintaining independence, safety, and quality of life at home. Our trained staff provide compassionate, medically-aware support that respects each person's dignity and follows their healthcare team's guidance.
We work closely with each individual's healthcare providers, service coordinator, and family to ensure every aspect of their care plan is followed consistently — so they can remain safely in the community rather than transitioning to a nursing facility.
👤 Who Qualifies?
Adults aged 18 and older with physical disabilities or chronic health conditions who are approved under Iowa's Health and Disability (HD) Medicaid waiver and require support to remain living independently in the community.
