What Are
Intellectual Disability Support Services?

Uni-Hope's Intellectual Disability (ID) program offers daily and hourly support services tailored to help adults with intellectual disabilities live meaningful, self-directed lives. We believe every person has unique strengths and the right to pursue their own goals — and our team is here to help make that happen.

We work hand-in-hand with each individual, their family, and their service coordinator to create a Person-Centered Support Plan that reflects their goals, interests, and needs — then we show up every day to help bring that plan to life.

SCL / Daily Support
Consistent daily care and supervision in the home or community, structured around the individual's Person-Centered Support Plan.
SCL / Hourly Support
Flexible hourly assistance with daily living tasks, errands, appointments, and community participation.
Life Skills Training
Hands-on instruction in cooking, budgeting, housekeeping, laundry, grocery shopping, and personal care.
Community Integration
Support to build relationships, explore recreational interests, access employment, and participate in community life.
Transportation Assistance
Safe and reliable transportation to appointments, employment, day services, and community activities.
Goal-Based Programming
Person-Centered Support Plan (PCSP) goal work with regular documentation and progress tracking.

Questions

Iowa's Intellectual Disability (ID) waiver provides Medicaid funding for community-based support services to help adults with intellectual disabilities live outside of institutional settings.

We accept Amerigroup Iowa, Total Care, Molina HealthCare, and Polk County Funding for ID waiver services.

After acceptance, your assigned Service Coordinator works with you, your guardian (if applicable), and your case manager to build a Person-Centered Support Plan aligned with your goals and waiver eligibility.
